You can earn up to $350 for taking healthy actions! 

Any covered adult (18 and older) can earn rewards from either Kaiser Permanente medical plan by completing the following activities:

• Complete the online total health assessment, Succeed®, to receive a $200 Visa® reward card.
• Complete up to three healthy lifestyle programs to receive an additional $50 Visa® gift card for each completed program. The healthy lifestyle programs are free, interactive, and personalized to empower you to take charge of your own health and reshape your lifestyle.

Wellness Coaches

Need some one-on-one support? Contact a wellness coach. Call 808-432-2260 to partner with a coach to help you make healthy habits a part of your daily routine.

Fit Rewards

Get rewarded for staying active and getting fit. Enroll in the Fit Rewards program, pay a $200 fee and work out at a participating gym at least 45 days for a minimum of 30 minutes per session between January 1 and December 31, and you’ll get your $200 back. You can even work out from the comfort of your own home.
