Dental Plan B Highlights

  • Plan B offers both in-network and out-of-network care. Read the DPPO Customer Brochure for more information and learn how to find a dentist in the DPPO plan.
  • Orthodontia benefits are available for children to age 19 only.
  • When you go to an in-network dental provider, you’ll pay less overall and there are no claim forms to file.
  • You pay coinsurance for in-network services.
  • When you go to an out-of-network provider, the plan pays benefits based on the reasonable and customary (R&C) charge for a particular service.  You’ll generally be asked to pay for your care at the time of your visit and submit a claim for reimbursement.
  • If the out-of-network provider charges more, you’ll be responsible for paying the amount above the R&C charge, in addition to applicable coinsurance and deductible.
