Long-Term Disability

Provides a monthly benefit that replaces a portion of your income.

If you suffer a disabling injury or illness, you may be unable to work for an extended period of time. Hertz offers you optional income protection coverage under the Long-Term Disability (LTD) plan. The plan pays you a percentage of your salary if you continue to be disabled and unable to work after exhausting your Short-Term Disability (STD) benefits.

How it Works

The LTD Plan replaces a portion of your basic monthly earnings if you become disabled due to illness or injury. You may elect to pay for LTD coverage with after-tax contributions or pre-tax contributions. When you elect to pay with after-tax contributions, your LTD benefit is provided without income tax. If you choose LTD coverage on a pre-tax basis, any benefit payable to you will be subject to income tax.

You are eligible to receive LTD benefit payments after you have been disabled for 180 days on an approved period of STD. Your benefit will be a percentage of your Base Pay at the time you became disabled, depending on the option elected, and subject to benefit minimums and maximums. Your LTD benefit will be reduced by benefits you receive, or are eligible to receive, from other sources such as Social Security and workers’ compensation.

Benefits will continue for the maximum benefit period, provided you remain disabled.

Filing a Claim

You may file a claim for disability benefits at any time after you know you will be absent from work for 7 consecutive days or more, due to an illness or injury.

You must provide proof of your disability when you file a claim.

You will receive disability pay for the period of your disability, minus the waiting period, if your claim is approved.

If your claim is denied, you have the right to appeal.

You may file a disability claim by telephone or on the web. Contact Sunlife at 1-877-424-0929 or file the claim online at the Sunlife website. Additional information can be found in the STD brochure.
